The Role

We are looking for an enthusiastic two Social Care Assessorsto join us in providing quality services for vulnerable adults in Herefordshire.

An exciting opportunity has arisen within the South Locality Team for a Social Care Assessor on a permanent basis, working 37 hours per week.

This rewarding role offers the opportunity to work with adults who have a wide range of needs, as well as their families and informal carers, to ensure they receive the appropriate care and support to achieve the outcomes that matter most to them.

What will you be doing?

Our Social Care Assessors in Adult Social Care are an essential part of our team, and the right candidate will have lots of opportunities for learning and potential career progression.

As a Social Care Assessor, you will undertake person‑centred, strengths‑based assessments, support planning, reviews, and risk assessments, focusing on maximising independence, promoting wellbeing, and supporting individuals to make informed choices about their lives.

You will work collaboratively with partner agencies, community resources, and families to develop creative and sustainable solutions that build on people's existing strengths and networks.

About You

You will have experience of working with adults with a range of physical disabilities, learning disabilities or mental health needs. An excellent communicator with honed organisational skills, you will be looking to further develop in a career that offers great progression and makes a difference to adults' wellbeing.

You will receive regular supervision, working in a supportive environment with other social care staff and have a range of benefits to ensure a good work-life balance while also being able to conduct the full requirements of the role.

Herefordshire is a rural county and the ability to travel is essential to the post. This is a full time post with the benefit of flexible hybrid working.
